Kindergarten Registration Information
You must have the following
to register your child:
1. PROOF OF THE CHILD'S AGE
Prior to admission
to an Arkansas public school, a parent or guardian must provide
a document indicating the child's age. A birth certificate, passport,
military identification or document certified by a county recorder
may be used. Children who will be 5 years old on or before September
1 may be enrolled in kindergarten.
2. THE CHILD'S SOCIAL
SECURTIY NUMBER
A parent or guardian must
furnish to the school the child's social security number prior
to admission to an Arkansas public school. If your child does
not have a social security number please contact the school for
assistance.
3. HEALTH
RECORD
In accordance with Arkansas
law, every pupil must present a health record certifying immunization
from polio, diphtheria, tetanus, pertussis, measles, rubella,
mumps, hepatitis B, and varicella. This certificate may be from
a licensed physician or a County Department of Health.
4. PROOF OF PHYSICAL
EXAMINATION
Before your child may
attend kindergarten, Proof of Physical Examination is required. All
enrolling kindergarten students must be evaluated with the Early, Periodic,
Screening, Diagnosis, and Treatment (EPSDT) or its equivalent within 2 years
prior to enrollment in kindergarten. The physician's fee for this examination
is the responsibility of the parent or guardian.
5. PROOF OF LEGAL RESIDENCE
IN THE STATE OF ARKANSAS AND THE SILOAM
SPRINGS SCHOOL DISTRICT
The Siloam
Springs School District is required to provide an educational program only for
those students who are legal residents of the district. An Arkansas driver's
license, rent receipt, or utility bill may be used.
